Camp and sports physicals

Everyone looks forward to the adventures of camp and the excitement of participating in summer sports. We'd like to provide some guidance for getting ready for camp or summer sports.

Forms

Most camps or sports activities require a completed physical activity or camp physical form. Bayshore Pediatrics can complete these forms as long as your child has a complete physical exam in the last year or two, depending on the requirements of the camp or sports activity (this will be noted on the form).

Wisconsin Interscholastic Athletic Association sports form

  1. All forms are completed and available for pickup (or can be mailed) 48 business hours from the day we receive the form. Please keep this in mind when preparing for camp.

  2. If your child does not have a current physical, please call to schedule an appointment. Keep in mind that most physical appointments are made at least three weeks in advance, if not more.
