Welcome. You have given us the greatest compliment by allowing us the honor of taking care of your child or children. Thank you for the confidence you have placed in us.
We invite you to call our office today to register your children as patients. Once this has been done, we can set up appointments for the needs of your children, whether it be a well physical exam or sick visit.
Your first appointment
Please arrive 15 minutes early in order to complete necessary forms.
Bring your current insurance information for each of the children.
Bring your vaccine record if at all possible.
Bring the completed registration and health history form.

Past medical records
New patients should make arrangements to have past medical records sent to our office.
