Our insurance personnel are available during weekday office hours to assist you with questions related to billing and insurance coverage for your child's visit.
We accept most commercial insurances, and currently are providers for many plans such as United Healthcare, Humana and Anthem Blue Cross. At the time of your office visit, a copy of your insurance card will be requested for our records. Should your insurance or home address change at any time, please keep us informed. Correct insurance information ensures timely payment of your bill. Because individual plans vary, it is the responsibility of our parents to know the coverage requirements of their insurance.
If your insurance requires a co-payment, it must be paid at the time of service to avoid any additional administration fees. Be advised that surcharges also may apply for an unscheduled walk-in or after-hours office visit.
Account statements are mailed to you monthly from our Centralized Billing Office (CBO) under the logo of Children's Medical Group. We are happy to submit your insurance claim for you and allow 60 days for insurance payments. After that, the balance becomes the patient's responsibility to pay promptly or investigate the reason for delay. Parents are advised to contact their insurance in the first 30 days to add their newborn to their plan.
We are happy to assist if your insurance requires a referral to another physician or a preauthorization for medical services. It is the parent's responsibility to contact our billing department to process this at least one week prior to the appointment.
Forest View Pediatrics frequently utilizes the Children's Hospital of Wisconsin lab and radiology services that are located in our building. While some lab tests are performed by our own clinic staff, occasionally these specimens are sent for further testing. Please note that lab and radiology services that are performed outside of our practice are billed separately by the Hospital.
