Medical Home is an approach to providing health care in a family-centered manner. As our population of children and youth with special health care needs continues to grow, Children's Medical Group-Southwest Pediatrics is focused on finding ways to evolve the Medical Home into a community of shared experiences, knowledge and resources. This quarterly newsletter provides families with needed information on transition to adult providers, community resources for multiple issues and parent trainings. We hope this newsletter will create an interactive community of families of children with special health care needs.
