H1N1 & seasonal flu vaccine clinic information

When can my child get vaccinated for H1N1?

We have a good supply of H1N1 vaccine and we are happy to administer this vaccine to all children 6 months and above. At the vaccine clinics, only Forest View Pediatrics patients may receive the vaccine. We thank you for your cooperation while we follow the CDC recommendations to vaccinate patients in the priority groups first. 

These clinics are for H1N1 and seasonal flu vaccine only.

  • Because supply levels have been changing frequently, we have needed to be creative about our scheduling strategies this year.  We appreciate your patience over the last few weeks and apologize for any inconvenience.  Supply is becoming more stable so we are now happy to schedule appointments in advance.
  • Our schedules (especially at the beginning and end of the day) are set up to try to meet the needs of as many patients for the vaccine as possible, so please be patient if there is a short wait for the vaccine.
  • We will schedule for either the first or second dose of the vaccines as long as our supply holds.
  • Our schedules are set up to try to vaccinate as many patients as possible, so we ask that you call ahead to speak to a triage nurse if you have any medical questions, rather than consult the immunization nurse.

Please call our office to obtain updated supply and scheduling information by pressing option 1 after hours, or option 7 during the day.  Our Web site also will be updated with supply and scheduling information.

Other vaccine clinic information:

  • Vaccine clinic dates and number of patients seen may change depending on supply levels and CDC guidelines.
  • You will be asked to complete a questionnaire and sign this as a consent.  You can print up the questionnaire/consent form below and bring it with you to your visit.

  • These clinics are with the nurse only and for H1N1 and seasonal flu vaccinations only. Appointments for questions with physicians must be scheduled for a separate day.
  • Other vaccines will not be given at these clinics (no exceptions). You will need to schedule a separate appointment for other vaccines.
  • Children younger than 18 must be accompanied by a parent or guardian.

What else do I need to know?

We are making every effort to meet the needs of our patients who wish to be vaccinated. If you have a significant number of questions or wish to have your child seen prior to a vaccination, we ask that you make a separate appointment with a physician.
