Prescription refills 

Prescription refills are handled during regular office hours so that your child's medical chart can be reviewed. We ask that you request your prescriptions be refilled at least 24 business hours in advance. To request a prescription refill, please call (414) 228-0099, option 3. We ask that you clearly provide the following information:

1. Your name (first and last).
2. Your child's name (first and last).
3. Your child's date of birth.
4. The telephone number we can call you back at.
5. Your pharmacy telephone number (please include area code) and name.
6. The name and strength of the medication you need refilled.

Prescriptions will not be refilled after regular hours unless it is an emergency. As a policy, we do not dispense new prescriptions over the phone without an office visit.
